Co-pays will be due at the time of service.
We offer multiple ways of paying for any outstanding balances. We encourage that you consider one of the following options:
- Payment plans:
- We would like to set up a payment plan for you as soon as your bill reaches $100 or more. We have options to pay weekly, bimonthly or monthly. A minimum of $25 or 20% is required.
- We also have an option to autodraft your payment plan until your balance is paid. The autodraft is available on the 1st and 15th of every month or on each Friday. Minimum of $25 or 20% of your balance is required.
This plan has a yearly fee of $49.00.
With this plan your first time visit will not exceed $125 and follow up visits will not exceed $55.
At Apple Chiropractic Orthopedics we set up a treatment plan that we believe will be the best for your condition. Often times insurance and our treatment plans to not coincide. Therefore, we encourage you to know your benefits and choose one of these options to ensure that payment will be made in a timely manner.
Any outstanding balances over 120 days without a payment plan or arrangement may be sent to a collection agency.
We believe in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. Insurance
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge.
No Insurance
For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have chiropractic benefits there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.
